Job Summary

Own the referral-network expansion for home care by building relationships with hospitals, physicians, and community partners, while ensuring steady intake, census growth, and local visibility.

Responsibilities include developing and maintaining referral relationships, visiting healthcare professionals, and representing the agency at community events to drive referrals.
Coordinate with clinical and operations teams to streamline intake and patient satisfaction while tracking marketing activity and census trends.
Identify new market opportunities and sustain a consistent territory presence with a defined call cycle to hit growth targets.

Job Description

Responsibilities

Develop and maintain relationships with hospitals, skilled nursing facilities, assisted living communities, physicians' offices, and other referral sources
Conduct regular in-person visits and presentations to healthcare professionals and community partners
Represent the agency at community events, health fairs, and networking functions
Collaborate with the clinical and operations team to ensure smooth intake and client satisfaction
Track and report on marketing activity, referral trends, and census data
Identify new market opportunities and develop strategies to increase agency visibility
Maintain a consistent presence in the territory and manage a defined call cycle
