Job Summary
Build and own a healthcare marketing and referral network for home care by winning Medicaid/Medicare case manager relationships, hospital and social-work partnerships, and private-pay sources to drive qualified referrals and higher census.
- Manage referral ecosystem with case managers, discharge planners, hospitals, and elder care partners.
- Educate sources on services, care model, and programs to sustain inbound referrals.
- Track activity, report outcomes, and align with intake for smooth onboarding.
Job Description
- Develop and maintain referral relationships with: Medicaid case managers
- Medicare case managers and discharge planners
- Hospitals, rehab facilities, and skilled nursing facilities
- Social workers and community agencies
- Private pay clients and family decision-makers
- Actively market Synergy Home Care services to professional and community partners
- Generate qualified referrals and convert leads into active clients
- Educate referral sources on Synergy’s services, programs, and care model
- Attend networking events, community meetings, and healthcare functions
- Track referral activity and report outcomes to agency leadership
