Own and grow a referral network as a Home Health & Hospice Liaison, driving patient referrals and educating healthcare partners while collaborating with clinical and administrative teams to smooth admissions.

Job Description
Responsibilities
- Build, develop, and maintain strong relationships with physicians, hospitals, skilled nursing facilities, assisted living communities, senior living communities, case managers, discharge planners, and other referral sources.
- Educate patients, families, and healthcare professionals to increase awareness, strengthen community presence, and drive patient referrals.
- Conduct regular in-person visits with referral sources to build trust, gather feedback, ensure satisfaction, and identify opportunities for additional services.
- Collaborate with clinical leaders, liaisons, management, administrative staff, and leadership teams to ensure a smooth admission process.
- Identify, evaluate, and pursue new leads based on market potential, geographic opportunities, and community needs.
- Represent Valerian Home Health and Hospice professionally throughout the community.
Requirements
- Previous experience in healthcare, home health, hospice, senior living, physician relations, case management, discharge planning, business development, marketing sales, or a related healthcare field.
- Strong relationship-building and customer service abilities with excellent communication skills.
- Professional, organized, and goal driven.
- Strong computer literacy and proficiency with Microsoft Office applications.
- Working knowledge of HIPAA regulations and healthcare compliance requirements.
- Reliable transportation required.
Preferred
- Bachelor's or Nursing degree preferred (Healthcare Administration, Marketing, Communications, Business, or related field).
- Established referral source relationships within the healthcare community.
- Proven track record of meeting or exceeding referral growth.
- Home Health or Hospice experience, a plus.
