Job Summary

Own and optimize population health digital outreach across SMS, email, IVR and patient portal to improve outcomes in diverse plans, while coordinating across care management, quality and provider ops and aligning with value-based care metrics.

  • Drive end-to-end digital outreach campaigns and campaign execution across channels, data-driven targeting, and compliance.
  • Align outreach with clinical workflows and quality measures in collaboration with analytics and care teams.
  • Scale digital engagement by moving manual outreach to automated channels and supporting future CRM capabilities.

Job Description

- The Population Health Digital Outreach Manager is responsible for designing, executing, and optimizing digital engagement strategies to improve health outcomes and quality performance across Medicaid, Medicare, D-SNP, and Commercial populations. - Working within an integrated delivery system, this role partners across the health plan, care management, quality, and provider operations to ensure outreach is coordinated, data-driven, and aligned with clinical workflows and value-based care goals. - This position supports both current-state campaign execution and future-state capabilities, including population health use cases for future-state CRM implementation. - Drive staffing efficiencies by transitioning appropriate outreach from manual phone calls to digital channels (SMS, email, portal, IVR) - Increase member participation in clinical, preventive, and care management programs through targeted digital engagement - Support scalable outreach models that reduce operational burden while maintaining or improving outcomes - Ensure vendor-supported digital communications align with PHP strategy - Own end-to-end population health digital outreach campaigns across SMS, email, IVR, and patient portal channels - Partner with the Analytics Organization to translate targeting outputs into outreach execution - Align outreach strategies with STARS, HEDIS, and state quality measures - Coordinate with care management and provider teams to align outreach with clinical workflows - Deploy campaigns across multiple tools while ensuring consistency and compliance - Support implementation and operationalization of population health CRM capabilities - Participate in requirements gathering, vendor evaluation, and user acceptance testing - Monitor campaign performance and drive continuous improvement - Ensure compliance with HIPAA, CMS, and communication regulations
