Own and lead rehab services at the facility as the primary Legacy representative, driving relationship-building, therapy program development, and patient-centered outcomes while coordinating with facility staff to meet standards.

Oversee staffing, scheduling, and collaboration with facility personnel to meet census-driven needs.
Develop and implement clinical programs aligned with outcomes and compliance.

Create rapport and positive working relationships with key facility personnel and fellow Legacy employees that allows for discovery of others’ needs.
Oversee internal marketing efforts to assist with relationship building with staff and residents while keeping community leaders apprised of Legacy’s programs and other offerings.
Manage site productivity and makes necessary changes to achieve budget.
Maintain a caseload that assists the rehab department in meeting standards.
Able to assess patient needs and develop clinical programs appropriate for the facility census.
Able to direct and train others in clinical service delivery across disciplines.
Daily coordination of rehab department with facility staff to plan and implement therapy programming.
Participates in clinical outcomes reporting and shares with the customer.

Requirements

Clinical background as a Physical Therapist (PT), Physical Therapy Assistant (PTA), Occupational Therapist (OT), Certified Occupational Therapy Assistant (COTA), or Speech Language Pathologies (SLP).
Three to five years’ experience in long-term care (LTC) or senior living.
Minimum of a degree from accredited college or university or equivalent experience (i.e., degree in PT, PTA, OT, COTA, SLP).
Licensed and registered (PT, PTA, OT, COTA, SLP) in state where assigned.

Preferred

Some prior supervisory experience preferred but not required.
